Identity Verification Document

1.Identity Verification Document of the principal person

You are required to submit any one set of the Identity Verification Documents listed in the table below (No. 1 - 6 below) for any type of request. All charges for necessary documents are at your own expense.

1 Copy of Japanese driver's license
and
copy of certificate of residence		 Must be valid and bear the name and current address.
Issued within the last 3 months.
2 Copy of Japanese passport
and
copy of certificate of residence		 Must be valid and bear the name.
Issued within the last 3 months.
3 Copy of health insurance certificate/card
and
copy of certificate of residence		 Must be valid and bear the name and current address.
Issued within the last 3 months.
4 Copy of pension book
and
copy of certificate of residence		 Must bear the name and current address.
Issued within the last 3 months.
5 Copy of long-term care insurance certificate
and
copy of certificate of residence		 Must be valid and bear the name and current address.
Issued within the last 3 months.
6 Copy of basic resident register card
and
copy of certificate of residence		 Must be valid and bear the name and current address.
Issued within the last 3 months.

*Please make the part indicating permanent domicile information obscured and unreadable when making a photocopy.

2. Documents verifying the relationship to the principal person.

1) Legal representative

A) As a legal representative of a minor;

  • A document to prove that you have the power of legal representative.
    Person with parental authority: a copy of the extract relating to the principal person from the family register
    Guardian: a copy of the extract relating to the principal person from the family register
  • A document to prove that you are a legal representative of the minor
    (Documents specified in above 1, 'Identity Verification Document of the principal person').

B) As a legal representative of an adult ward;

  • Statement of commencement of guardianship
  • A document to prove that you are a legal representative of the adult ward
    (Documents specified in above 1, 'Identity Verification Document of the principal person').

2) As a representative by power of attorney

  • Power of Attorney
    The registered seal of the principal person must be stamped and a certificate of seal impression (Inkanshomeisho) of the principal person must be attached.

*If the representative is to receive a response from CE, the Power of Attorney must clearly state to the effect that "the principal person authorizes the representative to receive the response to the request from CE." Absent such a statement, the response will be directly sent to the principal person.
